On behalf of Indiana’s Family and Social Services Administration, we at the Center for Survey Research at Indiana University are conducting this study of recipients of the Indiana PathWays MFP Program. A CSR interviewer will schedule a telephone interview to learn about your experiences in order to help FSSA understand how to improve service to recipients in the future.

Who is conducting this survey?
The Indiana Family and Social Services Administration (FSSA) has asked the Indiana University Center for Survey Research (CSR) to help conduct this survey, which is designed to obtain feedback from their recipients.

The survey is sponsored by the Indiana Family and Social Services Administration (FSSA).

What is the purpose of the survey? How will the data be used?
The purpose of this survey is to learn about your experiences receiving services from your home and community-based services program.
The data from this survey will be combined with other data without information that will identify you and will then be provided to FSSA’s PathWays Money Follows the Person Program so they can understand how to improve service to their recipients in the future.

How long will the survey take?
The survey will be conducted by telephone and takes about 30 minutes to complete.

How did you get my name and phone number? How was I chosen for this survey?
Everyone in Indiana PathWays Money Follows the Person Program is being contacted to participate in this study. In order to conduct this survey, FSSA provided the Indiana University Center for Survey Research with contact information.

Who will see my answers?
Your answers will be kept confidential and will be seen only by authorized people at the Indiana University Center for Survey Research who are conducting this survey on behalf of FSSA. All responses will be merged into a large pool of data that will be shared with FSSA, which is responsible for overseeing the services you receive.

Will my responses affect my benefits?
No. Your responses will not affect your benefits.

I am not able to complete a survey by myself. Can I have someone help me?
If you feel you are unable to complete the survey yourself, you can ask a family member or friend to help you. The person who helps you should be a family member or relative, but could also be an unpaid caregiver or a close friend. This person needs to be someone who knows you very well and would be able to help you answer the questions and/or be able to answer the questions accurately for you.

Who is the Indiana University Center for Survey Research?
The Center for Survey Research (CSR) is a part of Indiana University in Bloomington, Indiana. The CSR was established in the early 1980’s and conducts 35-50+ projects each year providing high-quality research data to state agencies, non-profit organizations, and researchers across a wide range of fields. CSR telephone interviewers complete rigorous training and are carefully monitored to ensure that they are conducting interviews accurately and in a professional manner at all times.
